Title: Incidence of ESBetaL producing Klebsiella pneumoniae among diabetic and non-diabetic patients from north-eastern part of Karnataka

Abstract: A total of 286 (1372) K. pneumoniae included in the study were recovered from clinical samples, 270 Urine (1288), 6 blood (36), 9 stool (45) and 1 sputum (1) indicating an isolation rate of 20.84%. Among them, 157 (54.89%) were from diabetic and 129 (45.10%) were from, Non-diabetic individuals. A very high incidence of resistance to the entire fourteen antibiotics ranging from 00.00% to 99.65% is observed with highest (99.65%) resistance against amphicillin and lowest (00.00%) resistance against Imipenem. The inicidence of Klebsiella pneumoniae in urine samples of the diabetic males was considerably higher than the non-diabetic males. However in females the incidence was marginally more in the non-diabetic individuals. Of the 286 K. pneumoniae isolates, 275 (96.15%) were positive for ES?L production. Multiple antibiotic resistances were most common and the majority of the isolates 129 (45.10%) were resistant to as many as seven antibiotics. All the isolates were sensitive to imipenam. Patients with diabetes mellitus (DM) have a higher prevalence of asymptomatic bacteriuria especially of K. pneumoniae, glucosuria can explain this increased incidence High percentage of drug resistance in ES?L producing K. pneumoniae suggests that monitoring of ES?L production and antimicrobial susceptibility testing are necessary to avoid treatment failure.
